Alcohol/Drug Usage Pre and Post Employment Testing Authorization Form for Maine Operations

 

As part of its policy to provide a safe and healthy work environment for its employees, The Davey Tree Expert Company and Subsidiaries (“Davey”) tests all applicants with a conditional offer of employment for drugs. If the drug test is confirmed as positive, the results will be considered in our decision to employ you and may result in a rejection of your application for employment.

You should also understand that Davey reserves the right to test employees for drug usage or possession where there is reasonable cause to believe that testing is necessary and further reserves the right, on Company property, to search any areas, including lockers, desks, cars, lunch boxes, and other areas, where permitted by law, as part of its policy.

I have read the foregoing statement and understand that the pre-employment screening process includes a specimen test which may disclose the use of drugs or alcohol, or which may indicate substance abuse or chemical dependency.

I consent to such a test and to the disclosure of the results of the test to Davey for its internal use only.

Pursuant to Maine law, Davey Tree does not test applicants for marijuana. However, the Davey Tree Substance Abuse Policy prohibits employees from possessing, using or being under the influence of marijuana while at work. Under the policy, Davey Tree will test employees for marijuana use in the event of reasonable suspicion of a violation of these requirements. I further consent, if hired, to Reasonable Cause alcohol/drug usage tests and to searches on The Davey Tree Expert Company property of lockers, desks, cars, lunch boxes and other areas which may belong to or be assigned to me (where permitted by law) in accordance with the Davey Tree Company's Substance Abuse Policy and the Department of Transportation Alcohol and Drug Policy, copies of which will be provided to me.
